Notice of voluntary 2 PIN EURO TO 3 PIN UK ADAPTOR replacement programme for users of Toshiba Dynadock V10 Universal USB Port Replicator
Toshiba is undertaking an end user replacement programme in respect of the 2 pin Euro plug to 3 pin UK plug adaptor (supplied with Dynadock V10: PA3778E-1PRP) prior to 1st December 2010.
During Toshiba’s on-going quality control monitoring of its products, it has come to our attention that there may be a remote possibility of injury being caused by the 2 pin Euro plug to 3 pin UK plug adaptors which were supplied with the power adaptors of the Dynadock V10 Universal USB Port Replicator (see Annex 1 for a photographic example of this product).
Toshiba believes that in the worst case scenario it would be possible for customers to receive an electric shock if a metal object or a finger was deliberately inserted into the 2 pin holes whilst the 2 pin Euro plug to 3 pin UK plug adaptor is plugged in and the electricity is switched on. This risk could only come about through deliberate misuse of the product. Whilst there have been no reported injuries and/or incidents, Toshiba is undertaking a replacement programme of the 2 pin Euro plug to 3 pin UK plug adaptors.

1. Affected Products
Toshiba’s replacement programme involves the 2 pin Euro plug to 3 pin UK plug adaptor contained in Toshiba Dynadock V10 Universal USB Port Replicator supplied prior to 1st December 2010.
2. Immediate action
Please stop using the 2 pin Euro plug to 3 pin UK plug adaptor immediately and contact Toshiba on the number provided below to arrange your replacement adaptor.
3. Steps to exchange the 2 pin Euro plug to 3 pin UK plug adaptor
To obtain your replacement adaptor, please contact Toshiba by telephone on the below numbers. You will be told how to return your 2 pin Euro plug to 3 pin UK plug adaptor and obtain a replacement.
The issue affects the 2 pin Euro plug to 3 pin UK plug adaptor only. The replacement programme is therefore only applicable in respect of the 3 pin adaptor and not the Dynadock v10 Universal USB Port Replicator itself.
